Stone Brook Assisted Living is designed for the resident who wants to maintain independence but may need some assistance.
Our care plans are designed to preserve and promote as much independence as possible.
We develop a plan of care to accommodate each resident. A resident may need help with some or all aspects of daily living every day, or just from time-to-time.
Therefore, comprehensive assessments result in a plan that allows us to provide the needed care for an all-inclusive rate*.
Our beautiful grounds in north Texas provide a comfortable and quiet setting.
Additionally, our location is quite convenient for our neighbors in southern Oklahoma.
The base rate includes, but is not limited to, these services:
*Stone Brook facilitates the doctor and beauty salon but the resident still pays for the service.*
*Higher care services will be provided for an additional monthly cost. We will arrange special care when needed. Examples include:
THHS Notice: A long-term care ombudsman helps residents of a nursing facility and residents of an assisted living facility resolve complaints. Help provided by an ombudsman is confidential and free of charge. To speak with an ombudsman, a person may call the toll-free number 1-800-252-2412.
